The practical procedures sign-off (PPS) form should be used for procedures overseen by audiologists (please refer to the curriculum for further information). The signed form can be uploaded to the ePortfolio personal library. 

Download the JRCPTB out of programme application form, complete and return with your CV, deanery application form (see your deanery regarding this) and appropriate OOP information (eg job description, research proposal, clinical activity summary) to the appropriate contact for your specialty/deanery/HEE local office. Please ensure you provide the dates of any OOP in DD/MM/YY format.
