Form of Faith
Candidates who have successfully completed all parts of the MRCP(UK) Examinations are required to submit the Form of Faith once confirmation of their pass result is published on this website. The Form of Faith is retained as a permanent record of a candidate’s agreement to abide by the Laws, Bye-Laws, Statutes and Regulations of the Royal Colleges of Physicians of the United Kingdom.
A copy of the Form of Faith will be posted with result letters and is also available to download from this page. Candidates should complete and return this as soon as possible once they receive confirmation of their final pass to ensure that they are elected to Membership.

How to complete the Form of Faith
Candidates must complete Section 1 of the form with their own details and ensure that they sign and date this where specified. The form must supported by a Proposer who can vouch for the candidate’s moral character and professional conduct. This Proposer should have worked with the candidate within the last 3 years and must hold FRCP(UK) or have held MRCP(UK) for at least 8 years. They must complete and sign Section 2 of the form.
Forms that are not fully completed cannot be accepted and will result in a delay to election to membership.
Where to send the Form of Faith
Candidates should submit their completed Form of Faith to the College of Entry through which they passed the PACES Exam if they are not intending to attend a New Members’ Ceremony. Please see below for addresses. Alternatively, if a candidate does intend to attend a Ceremony they should return the Form of Faith to the relevant College along with the Ceremony form by the appropriate closing date. Details of Ceremonies and associated application forms will be sent with result letters following a candidate’s completion of the MRCP(UK) Examinations.
Candidates are strongly advised to submit their completed Form of Faith as soon as possible once their pass in their final part of the examination is confirmed. Any delay in submission of a completed Form of Faith will result in a delay in the candidate being elected to MRCP(UK) and receiving their MRCP(UK) certificate. They may also be unable to attend the Ceremony of their choice.
Candidates who have completed and submitted their Form of Faith will have their names presented to the three Royal Colleges of Physicians of the United Kingdom as being elected to MRCP(UK).
Members of the Royal Colleges of Physicians of the United Kingdom are elected subject to the Laws, Bye-Laws, Statutes and Regulations of the Royal Colleges of Physicians of the United Kingdom. Copies of the Laws, Bye-Laws, Individual Statutes and Regulations relating to Members can be inspected on application to the respective Colleges.
