Form of Faith
Election to membership occurs when a candidate has passed all parts of the exam and when a Form of Faith has been received, verified and processed satisfactorily. Until this process has been completed doctors are not considered elected to membership. The Form of Faith is retained as a permanent record of a candidate’s agreement to abide by the Laws, Bye-laws, Statutes and Regulations of the Royal Colleges of Physicians of the United Kingdom.

How to complete the Form of Faith
Candidates are required to complete Section 1 of the form with their own details and ensure this is signed and dated where specified. The form must be supported by a Proposer who can vouch for the candidate’s moral character and professional conduct.
Proposer criteria:
- should have worked with the candidate within the last 3 years
- be a Member or Fellow of the Royal College of Physicians of Edinburgh, Royal College of Physicians & Surgeons of Glasgow or the Royal College of Physicians of London
- be in good standing
- must have held MRCP(UK) for at least 8 years.
The proposer must complete and sign Section 2 of the form. Candidates are advised to submit their completed Form of Faith to the College of Entry through which they passed the PACES Examination.
Candidates are strongly advised to submit their completed Form of Faith once it has been confirmed they have passed in the final part of the examination. Any delay in submission of a completed Form of Faith will result in a delay in the candidate being elected to MRCP(UK), obtaining their MRCP(UK) diploma and admission to the ceremony of their choice.
