Applicants may use the following forms to make a credit card payment or request a refund.

 Credit Card Payment Form

If you experience a payment problem whilst making a credit card payment online, you may submit the above form instead. The credit card form will be treated with confidence. Please submit the form to the Administrative Office which is processing your application.

 Refund Request Form

If a refund for an examination fee has been approved by the Administrative Office  that is processing your application, you can complete the above Refund Request Form and post it to your Administration Office.
